(The above 3 schools sent interviewers to BJ in December last year. I think they also interview in SH.)
You can request telephone interview without an application on file with
Michigan - on-line request http://www.bus.umich.edu/prostudents/mba/admissions/interviews.html
You can request interview when filling out your application with
Duke (there's an interview request form in the application packet) http://www.fuqua.duke.edu/admin/daymba/admissions/interviews.html
You can request interview after sending out our app with
UCLA - by email: alumni.interview@anderson.ucla.edu
Interview can only be accommodated for first 2 rounds applicants.

The following schools conduct interviews by invitation only.
NYU - NYU intends to interview all admitted students. (chance of admission after interview - 90%)
Harvard - Harvard interviews about 20% of the applicants (over 50-60% of those interviewed will be admitted).
Columbia - Columbia interviews about 50% of the applicants.
For these schools, the ground rule is if you are not invited for interview, your chance of admission is very small (1%). (No news=bad news)
